If you are a new OB patient to Fayetteville Woman’s Care, welcome! We are pleased you have selected our office for your medical care. Our doctors, nurses and staff are looking forward to providing you with a positive experience. On your first visit to Fayetteville Woman’s Care, we will give you a bag full of samples and information to help you during and after your pregnancy.

What You Need on Your First Visit

To make your first OB visit as smooth as possible, you can complete much of your paperwork online. Through our Patient Portal, you can securely complete your medical history questionnaire. That information will be immediately sent to our office, so you won’t have to worry about bringing a copy with you. If you don’t complete the medical history online, you can request that a medical history form be faxed or mailed to you and you can send it in before your appointment or bring it with you on your first visit and for gynecology exams.
For your first visit, you will need to bring:

1. A Completed HIPPA Notice of Privacy Practices
2. A Completed Financial Policy Agreement
3. A Picture ID
4. Your Current Insurance Card — You must bring your insurance card with you on every visit, or we will have to reschedule your appointment. Keep us informed of any changes in your coverage. We will assist you in preparation of insurance claims for obstetrical and surgical services. We will need to make a copy of your original card for future visits.
5. The name of your Preferred Pharmacy — As you progress through your pregnancy, we may need to write a prescription for you. Having the name of your preferred pharmacy will ensure a quick turnaround for your prescriptions. Most prescriptions can be hand-written or electronically sent immediately to your preferred pharmacy.

What to Expect on Your First Visit

It is very important to complete all of the forms prior to your first visit. If you do not have time to complete the downloadable forms, please arrive at least 15 minutes prior to your scheduled appointment time.

To ensure that everything goes smoothly with your insurance company, you will meet with a member of our Insurance Department during your first OB appointment with us. Our staff will review your benefits and set up a payment plan. It is important that you bring your current insurance information. If we cannot make a copy of your current insurance card, we will have to reschedule your appointment.

Your first prenatal visit is very important. It is during this meeting that we gather pertinent background information to save you time on your future prenatal visits as well as to order necessary tests based on your background information.

On your first visit, you will speak with our experienced OB counseling nurse, Miranda, RN. This is an exciting visit, introducing you to our office and allowing us the opportunity to learn about you and your expectations during your pregnancy. We also will give you a physical exam, initial bloodwork and an expected date of delivery. Our staff will provide you with lots of interesting material to make your pregnancy go as smoothly as possible. You will receive educational material including diet information, hospital policies and as many free samples as we can get our hands on. This appointment runs about two hours in length.

A full-time triage nurse, Kelli, RN, handles non-emergency medical issues. She is available during business hours to answer your medical questions. You may reach her by calling our office at 910.323.2103 and pressing extension 445.

Your OB counseling visit is ideally scheduled 7-9 weeks into your pregnancy.
